Issuance of Travel Clearance

Travel Clearance is a document issued by the Department of Social Welfare and Development (DSWD) Field Offices, whichever is applicable to a minor traveling abroad unaccompanied by any of the parents or those persons having parental authority and legal custody over the child.

WHO NEEDS TO SECURE A TRAVEL CLEARANCE?

WHO IS EXEMPTED FROM SECURING TRAVEL CLEARANCE?

  • A minor accompanied by the following:
    • Either or both of the minor’s parents, if legitimate minor;
    • The minor’s biological mother, if illegitimate minor;
    • The minor’s father who has been granted sole parental authority or custody by the proper court, if the minor is illegitimate;
    • The minor’s legal guardian;
    • The person, including one of the minor’s parents, who was granted sole parental authority or legal custody by the proper court, in which case the court decision should specifically include a statement to this effect, i.e., naming the person to whom sole parental authority or legal custody over the minor has been granted;
    • Minor’s adoptive parents if the minor is granted an adoption decree and certificate of finality.
  • A minor who is an immigrant or a permanent resident abroad, with a foreign passport, or whose parents are in the Foreign Service holding a diplomatic/official passport such as Ambassador/ General Consul/Consul/Attaches, provided he/she is holding a valid passport such as dependent visa/identification card which served as proof that the travel does not constitute trafficking.

For Minors Traveling Alone to a Foreign Country for the First Time

  1. Duly accomplished Application Form
  2. Photocopy of PSA authenticated Birth Certificate of Minor (Original and Photocopy)
  3. Any of the following documents or as necessary:
    • Photocopy of PSA authenticated Marriage Contract of parents (1 copy); or
    • Copy of court-issued Legal Guardianship/Tallaq or Fasakh Certification from the Shariah Court or any Muslim Barangay or religious leader; or
    • if born illegitimate, PSA authenticated Certificate of No Marriage (CENOMAR)
  1. LSWDO/SWAD Social Worker’s Assessment, when necessary
  2. Duly notarized Affidavit of Consent, accompanied by a copy of a valid ID featuring three specimen signatures of both parents or the person exercising parental authority or legal custody over the minor, as applicable.
  1. Duly notarized Affidavit of Support and certified copy of evidence to show the financial capability of the sponsor (e.g., Certificate of Employment, Latest Income Tax Return, Bank Statement)
  2. Two (2) pieces colored, and the latest passport-size pictures of minor (white background) taken within six (6) months
  3. Unaccompanied Minor Certificate
  4. Waiver from the parents releasing DSWD from any liability/responsibility in case of an untoward incident during the travel of the child.
  5. In the case of a deceased parent, PSA issued a Death Certificate (Original and Photocopy).

For Succeeding Travel of Unaccompanied Minor or Traveling Alone

  1. Duly accomplished Application Form
  2. Duly notarized Affidavit of Consent, accompanied by a copy of a valid ID featuring three specimen signatures of both parents or the person exercising parental authority or legal custody over the minor, as applicable.
  3. Duly notarized Affidavit of Support and certified copy of evidence to show financial capability of the sponsor (e.g. Certificate of Employment, Latest Income Tax Return, Bank Statement)
  1. Two (2) pieces colored, and the latest passport-size pictures of minor (white background) taken within six (6) months
  2. Unaccompanied Minor Certificate from the Airline
  3. Waiver from the parents releasing DSWD from any liability/responsibility in case of untoward incident during the travel of the child.
  4. In the case of a deceased parent, PSA issued a Death Certificate (Original and Photocopy)
  5. Original copy of the Previous Travel Clearance issued

For Minors Attending Conference / Competition / Educational Trips / Other Related International Activities

For abandoned minors with alleged missing parents

For minors who will study abroad

For minors migrating to another country

For Minors going abroad for medical purposes

For minors going abroad for inter-country adoption

For minors under Foster Care

For Minors under Legal Guardianship

For Minors who are the subject of an ongoing adoption case

For Minors Whose Parent/s are Seafarers

NOTE: Other documentary requirements may be required according to the purpose of travel and based on the social worker’s assessment.

The Travel Clearance Certificate may be obtained within three (3) working days upon submission of the required documents.
A fee of Php 300.00 per child with One (1) year validity

In case of loss of the issued travel clearance, a new set of requirements should be submitted to the Field Office, and a new travel clearance will be issued. This should not be taken as a renewal of travel clearance but as a new application.
